Now Itachi is using his most powerful technique and he is nearly out of chakra, so showing fatigue would be natural as we see here.

Now we see him clutch his chest in agony and judging by his face, it doesn't look like he anticipated this. This must've been excruciatingly painful for Itachi when looking at him doubling over like that while coughing up blood. Now, judging by Sasuke's and Zetsu's reactions, it seems that this was an unprecedented display of fatigue comparing to the previous pages.

So let's speculate. What is wrong with Itachi? And no, referring to Death Note is not allowed. :amuse
 
He used a technique with too low chakra and he's about dead becasue of it. He's over used his body and that's causeing it to pretty much shut down. I'd guess any way.
 
Susano's negative side effects is sucking all the life force out of him. Susano is probably a last resort jutsu thats why Itachi didn't use it until he had no other choice.

Thats it!

I guess such a high level jutsu takes its toll. Especially when you are done after using both eye techniques and the final one in one fight.
 
High cholesterol finally made its presence felt; Itachi had been eating too many cheeseburgers.

I expect Susano'o is just a technique that injures you hugely the more you use it; sort of like Amaterasu's uncomfortable tendency to make your eyes bleed buckets, except that it affects your whole body. He possibly just didn't beat Sasuke fast enough, and using Susano'o wore him down. Something like that. Or maybe Oro did it. :0000
 
Come on, how shallow can your posts be? Did you even read my first post? It can't be Susanoo alone, it's too unprecedented. And if Itachi was going to use a jutsu that he knew he'd collapse from, then that's just freaking stupid.

but he did cough blood after amaterasu too and it was just his 3rd MS jutsu...pre-ts he did 3 without problems;
 
I think it's side effects of the MS, considering how bad the effects of just amaterasu and tsukiyomi were by themselves, but I won't rule out something like a secret illness.
 
honestly, just like Sasuke couldn't get attacks to hit him, Itachi couldn't get past Sasuke plot shield. It just to much...
 
3 possibilities.
- heart attack (death note some people seem perfectly fine and suddenly BANG they have a heart attack <funny, but mostly THIRD attack kills human, previous two just make him a bit weaker.>)
- Plotton ! Illogical Death no Jutsu!
- maybe Itachi depleted his chakra, used his 100%, and we know human die when he has no chakra

That's the problem; Itachi should have been eating a balanced diet of lean fish/shark meat and vegetables. But since he didn't want to offend Kisame, he kept eating burgers and deep fried delicacies. Unfortunately for Itachi, high cholesterol doesn't care if you're a genius shinobi with great skills; it will still kill you.
