If Itachi wanted new eyes so badly, why didn't he just have his own kids. Sure, he would have had to wait a little longer, but then he could train them/ manipulate them, and eventually get his eyes. He could have a mini Uchiha factory...

I believe the MS is connected to best friends and then EMS to siblings.
How it's connected is debatable. Especially with the MS, since Kakashi has it but obviously didn't kill his best friend. There's also no real proof Madara or his brother killed thier best friends.
However, the only implied way of gaining the EMS seems to be through a sibling. Specifically, by taking their eyes and apparently killing them as well. Or at least that's what Itachi knows, which is all that really matters since it's his goal we're talking about.

I think if you rip out someone's eyes then they are most likely going to die of blood loss, also "no". You just have to know someone with Mangekyou Sharingan and rip there eyes out for EMS.

And now I've thought of this.... Oro is so obsessed with getting the Sharingan, and he is obviously very intelligent. If he knew that by eventually getting it he would also go blind, then why would he be so adamant about getting Sharingan.
